How to register in 6 easy steps
(The forms are on the right side of this page)

1. Purchase a D.I. Passport form shopdi.org
2. Fill out the registration form
3. Fill out the volunteer form
4. Fill out the conflict form
5. Email or mail these to the registrar
6. Send your registration fee to the registrar
   (Check, Cash of P.O.)

7.  (Optional) Appraiser Volunteer
Do you know of an enthusiastic DI supporter?  We are seeking Appraisers for the Tournament.  There are both scoring and non-scoring appraisers.  A non-scoring appraiser can be a time-keeper, check-in, form checker, etc.

You can register your teams in one of 3 ways

1. Download and Print the PDF document(s) below, fill-out your information (please print legibly), make 4 copies and mail them to the Region 12 Tournament Registrar.
2. Download the WORD document(s), edit the file, save it, print 4 copies and mail them to the Region 12 Tournament Registrar.
3. Download the WORD document(s) below, edit the file, save it, email it to the Region 12 Tournament Registrar.

(Please note:  In most cases when you download forms, your computer will place them into a temporary folder.  If you intend to edit/print any of these files, be sure to save them into YOUR document (or similar) folder)
